    Duodenal Carcinoma from a Duodenal Diverticulum Mimicking Pancreatic Carcinoma

    An 81-year-old man was found to have a pancreatic head tumor on abdominal computed tomography (CT) performed during a follow-up visit for sigmoid colon cancer. The tumor had a diameter of 35mm on the CT scan and was diagnosed as pancreatic head carcinoma T3N0M0. The patient was treated with pylorus-preserving pancreaticoduodenectomy. Histopathological examination showed that the tumor had grown within a hollow structure, was contiguous with a duodenal diverticulum, and had partially invaded the pancreas. Immunohistochemistry results were as follows:CK7 negative, CK20 positive, CD10 negative, CDX2 positive, MUC1 negative, MUC2 positive, MUC5AC negative, and MUC6 negative. The tumor was diagnosed as duodenal carcinoma from the duodenal diverticulum. Preoperative imaging showed that the tumor was located in the head of the pancreas and was compressing the common bile duct, thus making it appear like pancreatic cancer. To the best of our knowledge, this is the second report of a case of duodenal carcinoma from a duodenal diverticulum mimicking pancreatic carcinoma

    Ground-state properties of a triangular triple quantum dot connected to superconducting leads

    We study ground-state properties of a triangular triple quantum dot connected to two superconducting (SC) leads. In this system orbital motion along the triangular configuration causes various types of quantum phases, such as the SU(4) Kondo state and the Nagaoka ferromagnetic mechanism, depending on the electron filling. The ground state also evolves as the Cooper pairs penetrate from the SC leads. We describe the phase diagram in a wide range of the parameter space, varying the gate voltage, the couplings between the dots and leads, and also the Josephson phase between the SC gaps. The results are obtained in the limit of large SC gap, carrying out exact diagonalization of an effective Hamiltonian. We also discuss in detail a classification of the quantum states according to the fixed point of the Wilson numerical renormalization group (NRG). Furthermore, we show that the Bogoliubov zero-energy excitation determines the ground state of a π\pi Josephson junction at small electron fillings.Comment: 6 pages, 7 figure

    Association of serum leptin with inflammation

    Background : The roles of serum leptin in knee joint inflammation are unclear. The objective of this study was to identify any associations of serum leptin level with intra-articular inflammatory cytokine levels in acute arthritic and nonarthritic knees of mice. Methods : Acute arthritis was induced by intra-articular injection of 2% carrageenan. Three groups (leptin-deficient ob/ob, wild-type (WT) and high-fat diet (HFD)-fed WT) were made. Serum leptin and inflammatory cytokines in the infrapatellar fat pad and synovium were measured before and 24 hr after injection. Affected knee joints were excised for histology 24 hr after injection. Results : The HFD-WT group had significantly higher serum leptin than the ob/ob and WT groups before and after carrageenan injection. The HFD-WT group had significantly higher IL-1β and IL-6 in the infrapatellar fat pad and synovium than ob/ob and WT before injection but significantly lower IL-1β, IL-6 and TNF-α than the ob/ob group at 24 hr. Conclusions : Hyperleptinemia induced by a HFD is involved in low-grade intra-articular inflammation in nonarthritic knee joints. In contrast, leptin deficiency causes excessive intra-articular inflammation in carrageenan-induced acute arthritis. Leptin alleviates acute arthritis, while chronic hyperleptinemia is involved in low-grade inflammation in normal knee joints

    Sheet Dependence on Superconducting Gap in Oxygen-Deficient Iron-based Oxypnictide Superconductors NdFeAs0.85

    Photoemission spectroscopy with low-energy tunable photons on oxygen-deficient iron-based oxypnictide superconductors NdFeAsO0.85 (Tc=52K) reveals a distinct photon-energy dependence of the electronic structure near the Fermi level (EF). A clear shift of the leading-edge can be observed in the superconducting states with 9.5 eV photons, while a clear Fermi cutoff with little leading-edge shift can be observed with 6.0 eV photons. The results are indicative of the superconducting gap opening not on the hole-like ones around Gamma (0,0) point but on the electron-like sheets around M(pi,pi) point.Comment: 8 pages, 3 figure

    Screening of the Maturity Status of the Tibial Tuberosity by Ultrasonography in Higher Elementary School Grade Schoolchildren

    This study aimed to obtain screening data on the maturity status of the tibial tuberosity in schoolchildren of higher elementary school grades for risk management of Osgood-Schlatter disease (OSD). The maturity stages and cartilage thicknesses at the tibial tuberosity were determined by ultrasonography on the occasion of a school-based musculoskeletal examination for 124 grade 5-6 elementary schoolchildren, and their associations with the students\u27 demographic characteristics and OSD were examined. The time-dependent changes of the maturity status of the tibial tuberosity were also examined in grade 5 students (n = 26) by a longitudinal survey. The cross-sectional survey showed that the epiphyseal stage was reached in 89% of girls and 35% of boys. The girls who had experienced menarche (n = 28) were all in the epiphyseal stage and had a decreased cartilage thickness (p = 0.004, after adjusting maturity stages). Students with OSD (n = 5) were all girls in the epiphyseal stage, and only two of them had an increased cartilage thickness. During the longitudinal survey, a marked increase in cartilage thickness from the previous measurement was observed in three boys (without clinical symptoms) and a girl who newly developed OSD. Two students with OSD without chronic pain had thin cartilage. In conclusion, for schoolchildren of higher elementary school grades, the risk of OSD is higher among girls with the epiphyseal stage. Cartilage thickness may not contribute to the diagnosis of OSD, since thick cartilage is not very common in OSD. However, cartilage thickness may reflect the status of OSD
